Patient Rating

Perimenopause is the stage preceding menopause, and for many women it is the most difficult phase to interpret.

Periods may still be occurring, but the cycle becomes less predictable. Sleep may become more disrupted. Mood may feel less stable. Some women experience flushing, night sweats, anxiety, reduced concentration, headaches, fatigue or vaginal dryness. Others report a general sense that they do not feel as well as they did previously, without being sure why.

Because symptoms can develop gradually and may fluctuate from month to month, perimenopause is often less obvious than patients expect. It sits within the broader scope of women’s health assessment at Basuto. For broader menopause care, investigation and treatment, see menopause and HRT.

Common Symptoms

Symptoms that may indicate perimenopause

  • Irregular periods, heavier or lighter bleeding, skipped periods
  • Hot flushes and night sweats
  • Sleep disturbance
  • Anxiety, irritability and low mood
  • Reduced concentration and fatigue
  • Headaches and joint aches
  • Vaginal dryness and reduced libido
  • A general decline in resilience or wellbeing

Not all women experience the same pattern. In some, the main issue is menstrual change. In others, sleep, mood or cognitive symptoms are more significant.

Diagnosis

Why perimenopause can be difficult to recognise

Symptoms of perimenopause overlap with many other common issues, including stress, poor sleep, thyroid dysfunction, anaemia and depression. This means a symptom list alone is not enough.

A proper assessment helps establish whether the pattern is likely to be hormonal, whether another cause should be considered, and whether tests or treatment are appropriate.

Investigation

Is blood testing always necessary?

Not always. In some women, the history strongly suggests perimenopause. In others, blood tests may help clarify the picture or exclude another cause.

Where testing is relevant, this may include hormone blood tests or thyroid blood tests.
