Weight loss treatment should begin with proper clinical assessment, not a one-size-fits-all plan. Some patients want help because they have been gaining weight despite making sensible changes. Others are worried about blood pressure, cholesterol, pre-diabetes, sleep apnoea risk, joint pain, or the effect of weight on day-to-day health. Some want to discuss GLP-1 treatment such as Mounjaro or Wegovy. Others want a medical opinion before deciding whether medication is appropriate at all.

At Basuto Medical Centre, weight management is approached as a clinical service. That means your consultation is not limited to calorie targets or generic lifestyle advice. We look at the medical picture properly: weight history, appetite patterns, previous attempts at weight loss, current medication, relevant symptoms, cardiometabolic risk, and any evidence that a hormonal, metabolic, or behavioural factor may be contributing.

This matters because weight gain is not always straightforward. In some patients the main issue is excess calorie intake over time. In others, disrupted appetite signalling, insulin resistance, menopausal change, low mood, poor sleep, binge eating patterns, reduced mobility, or medication side effects play a significant role. Effective treatment begins by identifying which of those factors are actually relevant.

About the Practice

Who This Service Is For and How Appointments Work

This service is for adults who want a medical route into weight management rather than a commercial diet programme. It is appropriate if you are concerned about steady or central weight gain, considering GLP-1 medication and want a proper clinical review first, living with raised blood pressure, cholesterol, pre-diabetes or fatty liver risk, struggling with appetite control or repeated weight regain, or unsure whether medication is indicated and want an honest assessment before deciding.

Every standard GP consultation is 30 minutes. That gives your doctor time to take a proper history, understand what has been happening, review previous weight loss attempts, explain what they think may be influencing things, and talk through the next step. If you need baseline blood tests, treatment, a prescription, follow-up or specialist referral, these can often be arranged as part of the same visit.

You do not need to register with us to book, and you do not need to leave your NHS GP. Many patients use private GP care alongside NHS services when they want faster access, more time, or continuity with a named doctor.

GLP-1 TREATMENT

GLP-1 Treatment: Where It Fits

GLP-1 medicines have changed the conversation around medical weight management, but they are not interchangeable with general lifestyle advice and they should not be presented as a shortcut without proper review. These medicines can reduce appetite, help patients feel full sooner, and support clinically meaningful weight loss in selected patients. However, they still need careful prescribing, realistic expectations, side-effect counselling, and structured follow-up.

Not everyone is a candidate. Not everyone needs medication. Equally, some patients who have already tried sensible diet and exercise changes may benefit from a medical route rather than another cycle of short-term weight loss followed by regain. Where appropriate, we can discuss medicines including Mounjaro and Wegovy within the context of proper medical review, safety assessment, and ongoing monitoring.

UNDERSTANDING WEIGHT GAIN

Weight Gain Is Sometimes a Symptom

Some patients come to a weight clinic for treatment; others come because weight gain appears to be a symptom of something else. Menopausal transition can affect fat distribution, appetite, sleep, and body composition. In men, low testosterone, poor sleep, reduced activity, and metabolic risk can all contribute to central weight gain. Chronic stress, low mood, emotional eating, and disrupted routines may also play a significant role.

That is why this service links to broader clinical pathways. Patients who need a wider review can move through to a general GP consultation. Patients dealing with low mood or emotional eating may also benefit from counselling support. In those cases, the most useful starting point is often a broader clinical review rather than a weight-specific treatment plan.

Same-Day Access

Same-day weight loss appointments in Fulham

One of the main reasons patients choose a private GP is speed. If you have reached the point where you want proper medical support rather than trying another plan on your own, you may not want to wait weeks just to begin the process.

We keep same-day appointments available wherever possible so patients can be assessed promptly. If you need advice but cannot attend in person, an online appointment may also be appropriate. If blood tests, examination or prescribing review are likely to be important, we will advise you to come into the practice instead.

What patients usually find most useful is not simply faster access. It is having enough time for a thorough review of weight history, metabolic risk, medication suitability and realistic expectations before any treatment decision is made.

“What made the difference was that it felt properly medical. We talked through blood tests, risks, side effects, what treatment would involve, and whether it actually made sense for me rather than just selling me a prescription.”

Your Appointment

What happens at a weight loss consultation?

A weight loss consultation covers more ground than most patients expect. Here is what a typical appointment involves.

The consultation begins with your medical background and current concerns. We review weight history, previous treatments, eating patterns, sleep, activity, alcohol intake, mental health factors, and family history where relevant. We also review current medication, because certain medicines can contribute to weight gain or alter the suitability of weight-loss prescribing. If you are asking about GLP-1 treatment, the discussion should also cover gastrointestinal symptoms, pancreatitis history, gallbladder disease, pregnancy considerations, diabetes history, and any red flags that would make treatment unsafe or inappropriate.

Depending on the discussion, your GP may recommend baseline investigation before starting medication. This can include blood tests to assess glucose control, HbA1c, liver function, renal function, thyroid function, and lipids. For some patients, a broader review through health checks is the right next step. The next step may be further investigation, lifestyle support and monitoring, or medically supervised treatment including Mounjaro or Wegovy where clinically appropriate. Often, it is a combination.

Because appointments are 30 minutes as standard, there is usually enough time to ask questions, discuss options properly, and leave with a clearer understanding of what is happening and what to do next.

Continuity

Ongoing Weight Loss Care and Follow-Up

Weight management that involves GLP-1 medication requires regular clinical review, not just repeat prescriptions.

Patients often tell us they value being able to come back to a doctor who knows their history, understands what has already been tried, and can review progress over time rather than treating every appointment as a fresh start. That matters for GLP-1 treatment, blood-test review, medication tolerance, dose progression, side-effect monitoring, cardiovascular risk, and any weight-related issue that benefits from regular follow-up rather than one-off advice.

If you expect to use the practice more regularly, membership can make that continuity easier and more cost-effective. If you only need to be seen occasionally, you can simply book a pay-as-you-go appointment.

Assessment, Treatment and Follow-Up

Investigations, Prescribing and Referrals

Assessment is more useful when it connects directly to investigation and, where appropriate, treatment.

If your doctor thinks blood tests would help clarify the picture, we can arrange them quickly. If treatment is appropriate, a prescription can be issued. If you need a broader view of metabolic or cardiovascular health, we can connect this with health screening, including a Well Man Check or Well Woman Check where appropriate. If specialist input is needed, we can prepare a referral. If follow-up is needed, this can be organised through the same practice.

This keeps the clinical pathway coherent. Baseline blood tests, prescribing decisions, dose adjustments and ongoing monitoring sit within a single clinical pathway. Weight management often connects to broader clinical questions — low testosterone, metabolic risk, or mood — and having assessment, prescribing and follow-up available through the same practice avoids the delays that come from coordinating between separate providers. Where emotional health is part of the picture, counselling may also be appropriate.

Common Questions

Frequently asked questions

Do you prescribe Mounjaro or Wegovy?

Yes. Mounjaro and Wegovy may be prescribed where clinically appropriate after assessment, including review of BMI, medical history, current symptoms and baseline blood tests where needed.

Will I need blood tests before starting treatment?

Usually, yes. Baseline blood tests can help check markers such as thyroid function, liver and kidney function, diabetes risk and wider metabolic health before treatment begins.

Is ongoing monitoring included?

Yes. GLP-1 treatment requires clinical oversight with regular follow-up. Membership can make ongoing monitoring and review more straightforward if you expect to need repeat appointments.

Can I discuss weight loss in an online appointment?

Yes. Many weight loss concerns can be discussed in an online appointment, especially as a first conversation. If blood tests, examination or prescribing review are likely to be important, we may advise you to attend in person.

What does the first appointment involve?

Your first appointment includes a full GP review of your weight history, symptoms, previous attempts, relevant medical background, and whether treatment, testing or further monitoring may be appropriate.

How much does a weight loss consultation cost?

A standard 30-minute GP consultation costs £150. If blood tests, prescriptions or follow-up are needed, your GP will explain the next steps clearly during the appointment.

Do I need to register with the practice first?

No. You do not need to register before booking, and there is no catchment area or waiting list.

Can you help if I am not sure medication is right for me?

Yes. A lot of patients book because they want proper medical advice before deciding. The appointment is not just about prescribing — it is about understanding whether treatment makes sense, what the alternatives are, and what the risks and benefits look like for you.

Can weight loss treatment link to other health concerns?

Yes. Weight gain often overlaps with broader issues such as cardiovascular risk, prediabetes, hormone concerns, fatigue or metabolic symptoms. That is one reason we often connect weight loss consultations with health screening, women’s health or men’s health where relevant.

What is the difference between pay-as-you-go and membership for weight loss?

Pay-as-you-go is best for occasional use. Membership may suit patients who want more regular access, continuity with a named GP, and lower overall cost if they expect to use the practice more often — particularly useful for ongoing GLP-1 monitoring.
