GLP-1 Side Effects: What Patients Should Know Before Starting

The most frequently discussed side effects of GLP-1 medicines such as Mounjaro and Wegovy are gastrointestinal. Patients may experience nausea, reduced appetite, early satiety, reflux, bloating, constipation, diarrhoea, or intermittent vomiting. For many people these effects are mild, predictable, and settle as the body adjusts — particularly during the dose-titration phase when the dose is being increased gradually. For others, symptoms are more persistent and may limit tolerance or require a dose adjustment.

Patients should also understand that side effects are not only about comfort. Poor oral intake, dehydration, significant nausea, and prolonged gastrointestinal upset can become clinically relevant — particularly if fluid intake drops or weight loss is too rapid. The aim is not weight loss at any cost. It is effective, safe, and sustainable treatment.

Common gastrointestinal side effects

Nausea is the most commonly reported side effect, particularly during the early weeks of treatment and when the dose is being increased. It tends to improve as the body adjusts, but some patients find it persists at higher doses. Constipation is also common and may need dietary adjustment or a simple laxative. Diarrhoea, bloating and reflux occur in a smaller proportion of patients. Reduced appetite is both a therapeutic effect and, at higher intensities, a potential concern if it leads to inadequate nutrition or dehydration.

Practical measures that can help include eating smaller meals more frequently, staying well hydrated, avoiding very fatty or rich foods during titration, and being patient during the adjustment period rather than assuming the worst. Your GP can advise on managing symptoms and whether a dose hold or slower titration is appropriate.

Less common but important side effects

Pancreatitis is a rare but serious risk that patients and prescribers should be aware of. Symptoms include severe abdominal pain radiating to the back, nausea, and vomiting that is distinctly different from mild treatment-related nausea. If this occurs, treatment should be stopped and medical advice sought immediately. Gallbladder problems, including gallstones, can also occur with rapid weight loss and are another reason for clinical monitoring. Injection-site reactions are usually minor. Changes in heart rate have been reported in some patients, which is one reason baseline cardiovascular assessment and ongoing review matter.

Both Mounjaro and Wegovy are started at a low dose and increased gradually over weeks. This is not optional — it is designed to reduce the frequency and severity of gastrointestinal side effects. Skipping doses or starting at a higher dose increases the risk of intolerable symptoms. A GP who is monitoring your treatment can advise on the right pace of titration based on how you are responding, whether to hold at a dose for longer, or whether the target dose needs to be adjusted.

Why monitoring matters more than the prescription

The difference between responsible GLP-1 prescribing and a transactional online service is what happens after the prescription is issued.
