Understanding ADHD

More than difficulty concentrating

ADHD-related difficulties in adults are often described in terms of concentration, but the wider picture is usually more complex. Patients may be struggling with organisation, emotional regulation, overwhelm, restlessness, procrastination, poor sleep, work strain or repeated cycles of burnout. Some have a previous diagnosis. Others are only beginning to ask whether ADHD could be part of the picture. In adults, it often presents less as hyperactivity and more as difficulty sustaining focus, managing time, completing tasks, regulating emotions and maintaining structure in everyday life.

At Basuto Medical Centre, patients who suspect they may have ADHD — or who have a diagnosis and are looking for ongoing support — can access GP guidance on assessment routes and counselling for the emotional and practical difficulties that commonly accompany ADHD.

Common Signs in Adults

How ADHD commonly presents in adults

In adults, ADHD commonly presents as difficulty sustaining attention on tasks, poor time management, forgetfulness, procrastination, restlessness, impulsive decision-making, emotional dysregulation, difficulty following through on plans, overwhelm with administrative demands, and a pattern of underperformance relative to ability.

Many adults with ADHD have developed coping strategies that mask the condition for years, only seeking assessment when demands increase — a new job, a relationship, parenthood, or a period of burnout that overwhelms existing coping mechanisms. Where symptoms overlap with other concerns, a private GP review can help clarify the picture before pursuing formal assessment.

Assessment Routes

How ADHD assessment works

Formal ADHD diagnosis requires specialist assessment — typically by a psychiatrist or clinical psychologist with experience in adult ADHD. A GP cannot diagnose ADHD directly, but a private GP consultation is often the most practical first step. It provides an opportunity to discuss symptoms, rule out other contributors (such as thyroid dysfunction, sleep disorders or mood conditions), and receive guidance on the most appropriate assessment route.

At Basuto, the GP can provide a referral letter to a specialist ADHD assessment service, advise on NHS and private pathways, and arrange any preliminary investigations — such as blood tests — that may be helpful before or alongside the assessment process. ADHD commonly coexists with anxiety and depression, which can complicate the picture and benefit from parallel support.

When to Seek Help

When ADHD concerns need professional input

It is worth speaking to a GP about ADHD when:

  • difficulty with focus, organisation or time management is consistently affecting work or daily life
  • you frequently start tasks but struggle to complete them
  • impulsive decisions or emotional reactions are causing problems
  • you feel persistently overwhelmed by routine demands
  • relationships are strained by forgetfulness, inattention or restlessness
  • you suspect ADHD but are unsure how to access assessment
  • existing coping strategies are no longer sufficient

A GP consultation does not replace specialist assessment, but it is often the most practical way to clarify symptoms, rule out other causes, and understand what support or referral is most appropriate.

How We Can Help

Private ADHD support at Basuto

At Basuto Medical Centre, ADHD support is available through GP consultations and private counselling. The GP can help clarify symptoms, discuss whether ADHD assessment is appropriate, arrange a specialist referral, and provide ongoing medical support alongside the assessment and post-diagnosis process.

Counselling can help with the emotional and practical difficulties that commonly accompany ADHD — including frustration, low self-esteem, relationship strain, work-related stress, procrastination and overwhelm. This support is available whether or not a formal diagnosis has been made.

ADHD frequently overlaps with other conditions. If relevant, support is also available for anxiety, depression, trauma, and addiction.

Your Appointment

What to expect

A GP appointment for ADHD concerns provides time to discuss what you have been experiencing, how long symptoms have been present, how they are affecting your daily life, and what you are hoping support may help with. The GP will consider whether ADHD is a likely explanation, whether other factors may be contributing, and what the most appropriate next step is.

This may involve a referral for specialist assessment, blood tests to rule out other contributors, counselling for related difficulties, or a combination of these. The aim is to give you a clear, practical route forward.
