Why preventive review becomes more worthwhile after 40, and what a sensible men’s health check should include.

The forties are when cardiovascular risk, metabolic health, and hormonal changes begin to have a more visible clinical impact for many men. Blood pressure may be drifting upward. Cholesterol and triglycerides may have changed. Blood sugar may be trending towards pre-diabetic levels. Weight — particularly around the abdomen — may be increasing despite similar habits. Sleep may be lighter, recovery slower, and energy lower than a decade ago. None of these changes necessarily indicate a serious diagnosis, but together they represent the point at which preventive review becomes genuinely worthwhile.

What to include

A structured over-40 health check should include blood pressure measurement, lipid profile (total cholesterol, HDL, LDL, triglycerides), HbA1c or fasting glucose (diabetes screening), liver function (particularly relevant for men who drink regularly), kidney function, thyroid function, and a full blood count. For men with relevant symptoms — fatigue, reduced libido, erectile changes, or central weight gain — testosterone assessment may also be worthwhile. PSA (prostate-specific antigen) should be discussed for men over 50, or earlier if there is a family history of prostate cancer.

Beyond blood tests, the consultation should review cardiovascular risk factors including family history, smoking, weight, exercise, and alcohol intake, as well as blood pressure trend and overall lifestyle — because blood results in isolation tell only part of the story. This is particularly useful for men who feel broadly well but want a more deliberate review, as well as for those with family history, work stress, reduced fitness, or symptoms that are beginning to affect confidence or function.

Common questions

Which health-check markers matter most for men after 40?

Blood pressure, cholesterol, fasting glucose or HbA1c, liver and kidney function, thyroid, and PSA where relevant are commonly reviewed. Testosterone is added where symptoms suggest a hormonal contributor. The exact panel depends on age, family history, symptoms and risk profile rather than being a fixed list.

How does a men’s health review differ from a general health check?

A men’s health review adds focus on prostate, testosterone, sexual health and metabolic factors that are more commonly relevant to men, while still covering the cardiovascular and metabolic core of a standard health check. Both can be reasonable starting points depending on what you want reviewed.

Is testosterone testing part of a standard men’s health review?

Not by default. Testosterone is added where symptoms or risk factors make it useful — low energy, reduced libido, erectile changes, sleep disturbance, central weight gain, or family history — and is typically taken fasting in the morning for an accurate result.
